SIMPONI (golimumab - generic name), manufactured by Centocor Ortho Biotech Inc., is newly approved by the FDA as the first monthly, self-injectable, anti-TNF-alpha therapy medication used to treat adults with the following medical conditions and is available currently through all BioScrip Specialty Pharmacies:
Rheumatoid Arthritis
SIMPONI™, in combination with methotrexate, is indicated for the treatment of adult patients with moderately to severely active rheumatoid arthritis.
Psoriatic Arthritis
SIMPONI™, alone or in combination with methotrexate, is indicated for the treatment of adult patients with active psoriatic arthritis.
Ankylosing Spondylitis
SIMPONI™ is indicated for the treatment of adult patients with active ankylosing spondylitis.

Recommended Dosing:
SIMPONI™ is administered by 50 mg subcutaneous injection once a month
- SIMPONI™ is intended for use under the guidance and supervision of a physician. Patients may self-inject with SIMPONI™ after physician approval and proper training
Two Delivery Options:
- 50 mg/0.5 mL single dose SmartJect™ autoinjector
- 50 mg/0.5 mL single dose prefilled syringe

Healthcare professionals interested in initiating a patient on SIMPONI™ (golimumab) therapy, please follow these steps to enroll a patient:
Physician's can submit an order for SIMPONI™ (golimumab) to our National Mail Service or a BioScrip Community Pharmacy.
To submit an order to our National Mail Service:
1. Download and complete the appropriate SIMPONI™ (golimumab) Therapy Initiation Form for:

2. Fax the form to 877-254-3674

To submit an order to a BioScrip Community Pharmacy:
1. Complete a SIMPONI™ (golimumab) "Therapy Initiation form". This form can be found on the BioScrip physician ordering page. Follow Step 1: “Choose an enrollment form” from the drop down menu. Print and complete the SIMPONI™ (golimumab) form.
2. Choose a BioScrip ordering location. A physician may choose to have the medication delivered from one of our local and regional pharmacy locations. Download the fax cover sheet for your designated location from the BioScrip physician ordering page. Follow Step 2: "Choose a BioScrip ordering location. If you need assistance choosing a pharmacy location, visit our Store Locator for location and pharmacy information.
3. Fax the BioScrip "Therapy Initiation Form" along with the "BioScrip Fax cover sheet" to the fax number indicated on the BioScrip fax form.
